Before we proceed with nutrition coaching, please take a few moments to complete the following forms. Once you begin a form, you will not be able to save and come back to it at a later time. Please allow 15-20 minutes to complete the forms. The information you provide to us is very important and will be used to develop a program that is best suited for your needs and achieving your goals. All information you provide will remain confidential.
Once the forms have been completed you will be able to schedule a consultation. During that time we will review your answers and cover any questions you have about the program or process. Complete a 3-day Food Journal. You may do so using the FOOD JOURNAL ONLINE ENTRY fill out a Word document and submit via the Online Entry, or you may use the MyFitnessPal app or website and share your Food Journal with Steph. Just add ‘StephRitterSP’ as a ‘Friend’ and select ‘Friends Only’ under Diary Sharing (select Settings then Diary Settings to modify Diary Sharing). After you’ve completed these steps, send Steph an email with a list of the dates to review.
Once the above steps are complete, you will be able to schedule your Nutrition Consultation. Please schedule your Nutrition Consultation to occur no sooner than 48 hours after you’ve submitted all of the required materials.SCHEDULE
